About eFiche
eFiche is a Rwanda-based digital health company building the AI services layer that powers clinical decision support, ambient transcription, risk flagging, and clinical summaries for nurses and midwives working across Rwanda’s primary health care system. We receive structured clinical data from health facility workflows, process it through AI models, and return actionable outputs that support better, safer care decisions at the point of consultation.
As our feature set grows and usage scales across more facilities and clinical domains, we are looking for a skilled AI Services Backend Engineer to build and harden the server-side foundation of our AI platform.
Purpose of the Role
The AI Services Backend Engineer builds and maintains the server-side logic of the AI platform — the services that process clinical data, orchestrate AI model calls, format outputs, and communicate reliably with national health system integration points.
Key Responsibilities
- Design, develop, and maintain the backend services and APIs that power eFiche’s AI features across transcription, clinical decision support, risk flagging, and clinical summaries
- Build and manage data pipelines that receive structured clinical inputs, process them through AI models, and return outputs in the correct format
- Coordinate with integration partners on API specifications, payload structures, and endpoint behaviour
- Ensure data integrity, security, and performance across all AI backend services
- Implement error handling, logging, and monitoring to ensure AI service reliability at scale
- Write and maintain backend test suites; support QA testers in reproducing and resolving issues
- Contribute to technical documentation and knowledge transfer materials for the Ministry of Health
Required Profile
- 5+ years of backend engineering experience
- Strong proficiency in at least one backend language (Node.js, Python, Java, or equivalent)
- Solid experience with REST API design and consumption
- Experience with relational databases (PostgreSQL or equivalent) and data modelling
- Familiarity with AI/ML service integration (calling model APIs, handling model outputs) is a strong advantage
- Understanding of health data privacy principles (de-identification, role-based access) is an advantage
- HL7 FHIR familiarity is an advantage but not required
